East Grand Forks Woman Faces 5 Medicaid Fraud Counts<\/h2>\n

East Grand Forks<\/strong> (trfnews.i234.me<\/a>) \u2014 Jessica Nicole Wavra faces five felony theft charges in Polk County District Court.<\/p>\n

A criminal complaint alleges Wavra submitted false service records while working for Alluma Inc. in Crookston.<\/p>\n

The company was formerly called Northwestern Mental Health Center.<\/p>\n

The alleged conduct happened between June 2020 and March 2023.<\/p>\n

The complaint involves Medicaid-funded mental health and case management services.<\/p>\n

Prosecutors allege the false records led to $29,213.27 in Medicaid payments.<\/p>\n

They say Wavra received $10,578.97 from those payments as wages.<\/p>\n

Audit Leads to Investigation<\/h2>\n

According to the complaint, Alluma reviewed Wavra\u2019s progress notes during an internal audit.<\/p>\n

The company compared reported telehealth sessions with work phone and Zoom records.<\/p>\n

Alluma found many reported sessions lacked matching phone calls or Zoom meetings, the complaint states.<\/p>\n

The company then referred the matter for further review.<\/p>\n

The complaint says Alluma later terminated Wavra.<\/p>\n

The Minnesota Attorney General\u2019s Medicaid Fraud Control Unit<\/a> investigated the allegations.<\/p>\n

Investigators interviewed service recipients and reviewed records, phone logs, Zoom logs, and payment history.<\/p>\n

Several recipients said appointments were often canceled, shortened, or never held.<\/p>\n

Some denied receiving telehealth sessions listed in Wavra\u2019s progress notes.<\/p>\n

One reported 60-minute session lasted only five minutes, according to investigators.<\/p>\n

Other sessions had no matching call or Zoom activity.<\/p>\n

More Than 480 Hours Questioned<\/h2>\n

The complaint alleges Wavra reported more than 480 hours that did not occur as documented.<\/p>\n

It says the claims involved services for more than 40 recipients.<\/p>\n

Alluma billed Minnesota\u2019s Medical Assistance program<\/a> and managed care companies using the submitted notes.<\/p>\n

Those companies included Blue Cross Blue Shield, HealthPartners, and UCare.<\/p>\n

Wavra told investigators that some service times may have been entered incorrectly.<\/p>\n

She reportedly had no explanation for sessions lacking any matching communication records.<\/p>\n

Five Felony Counts Filed<\/h2>\n

The first two counts allege theft by false representation involving public funds.<\/p>\n

The remaining three counts allege theft by false representation exceeding $1,000.<\/p>\n

Each felony count carries a maximum five-year prison term, a $10,000 fine, or both.<\/p>\n

The charges fall under Minnesota\u2019s theft statute<\/a>.<\/p>\n

The case is filed as 60-CR-26-849 in Minnesota\u2019s Ninth Judicial District.<\/p>\n

The complaint contains allegations, not findings of guilt. Wavra is presumed innocent unless proven guilty in court.<\/em><\/p>\n
