State Registrar Molly Crawford explains that individuals can request their original birth records by filling out a request form and submitting it to the Minnesota Department of Health Office of Vital Records. Under the new law, a non-certified copy of the original birth record will be issued, containing information such as birth parent names, the individual’s name, place of birth, and date of birth. There is a $40 fee to receive these birth records. Additionally, parents who placed a child up for adoption can fill out a preference form to indicate their desire for contact with the adopted person.
