  1. The failure was caused due to a engineering flaw in the bridge's connecting panels called "gusset" plates, which hold bolts that join the I beams and girders together. They were found to be half their required thickness of 1 inch (2.54 cm) – plates were 0.5 inch or 1.25 cm, and neglected corrosion. The gusset plates buckled and failed, bringing down the bridge.
