East Grand Forks Woman Faces 5 Medicaid Fraud Counts
East Grand Forks (trfnews.i234.me) — Jessica Nicole Wavra faces five felony theft charges in Polk County District Court.
A criminal complaint alleges Wavra submitted false service records while working for Alluma Inc. in Crookston.
The company was formerly called Northwestern Mental Health Center.
The alleged conduct happened between June 2020 and March 2023.
The complaint involves Medicaid-funded mental health and case management services.
Prosecutors allege the false records led to $29,213.27 in Medicaid payments.
They say Wavra received $10,578.97 from those payments as wages.
Audit Leads to Investigation
According to the complaint, Alluma reviewed Wavra’s progress notes during an internal audit.
The company compared reported telehealth sessions with work phone and Zoom records.
Alluma found many reported sessions lacked matching phone calls or Zoom meetings, the complaint states.
The company then referred the matter for further review.
The complaint says Alluma later terminated Wavra.
The Minnesota Attorney General’s Medicaid Fraud Control Unit investigated the allegations.
Investigators interviewed service recipients and reviewed records, phone logs, Zoom logs, and payment history.
Several recipients said appointments were often canceled, shortened, or never held.
Some denied receiving telehealth sessions listed in Wavra’s progress notes.
One reported 60-minute session lasted only five minutes, according to investigators.
Other sessions had no matching call or Zoom activity.
More Than 480 Hours Questioned
The complaint alleges Wavra reported more than 480 hours that did not occur as documented.
It says the claims involved services for more than 40 recipients.
Alluma billed Minnesota’s Medical Assistance program and managed care companies using the submitted notes.
Those companies included Blue Cross Blue Shield, HealthPartners, and UCare.
Wavra told investigators that some service times may have been entered incorrectly.
She reportedly had no explanation for sessions lacking any matching communication records.
Five Felony Counts Filed
The first two counts allege theft by false representation involving public funds.
The remaining three counts allege theft by false representation exceeding $1,000.
Each felony count carries a maximum five-year prison term, a $10,000 fine, or both.
The charges fall under Minnesota’s theft statute.
The case is filed as 60-CR-26-849 in Minnesota’s Ninth Judicial District.
